Bridges Health offers doctors, discharge planners, case managers, and providers a single access point for all communities statewide. You can call, text, or fax referrals 24/7/365. Our team of admissions specialists and hospital liaisons streamline paperwork and conduct bedside assessments at most major hospitals. Our facilities participate in provider meetings to stay attuned to your needs, working in partnership to ensure a seamless transition from the hospital to the post-acute environment and then home. Residents and Families
Many people come to us after a life-changing event such as surgery, a stroke, a heart attack, illness, or an accident. Bridges Health offers Skilled Nursing, Skilled Therapy, and Long-Term Care at 33 locations throughout Oklahoma. We offer 3 Memory Care Assisted Living residences and two Medicaid Memory Care units. Our team of certified Physical, Occupational, and Speech Therapists and clinical staff work to help you regain independence, mobility, and function. Long-term care residents receive nursing care and assistance with the activities of daily living while enjoying a broad array of social opportunities.
